I understand that my participation in any workshop or event offered by Lila Volkas Nutrition LLC (the "Company") is entirely voluntary.
I represent that I am in adequate health to participate in the workshop and that my participation does not pose an unreasonable risk to myself or others. I understand that workshops involve the preparation and consumption of food and beverages.
I understand that workshop ingredients may include common allergens including tree nuts, peanuts, gluten, dairy, eggs, soy, shellfish, and sulfites. I have reviewed or have had the opportunity to review the ingredient list for my workshop and have determined that participation is appropriate for my dietary needs. I confirm that I do not have a life-threatening food or beverage allergy that would be triggered by participation in this workshop, or if I do, I accept full responsibility for managing that risk and have consulted with the Company in advance.
I acknowledge that participation in cooking and food preparation activities involves inherent risks, including but not limited to risks associated with food handling, allergen exposure, and the use of kitchen tools and equipment. I voluntarily assume all such risks.
I, on behalf of myself and my heirs, successors, and assigns, hereby release, discharge, and hold harmless Lila Volkas Nutrition LLC and its owners, officers, employees, agents, contractors, and insurers (collectively, the "Released Parties") from any and all claims, damages, liabilities, losses, costs, and expenses, including attorneys' fees, arising out of or resulting from my participation in any Company workshop or event, including any injury, illness, allergic reaction, foodborne illness, or other harm resulting from my participation or my own conduct.
I acknowledge that California Civil Code Section 1542 provides: "A general release does not extend to claims that the creditor or releasing party does not know or suspect to exist in his or her favor at the time of executing the release and that, if known by him or her, would have materially affected his or her settlement with the debtor or released party." I expressly waive the protections of Section 1542 and any similar law, meaning this release applies to claims I am not currently aware of as well as claims I am aware of.
This release applies to all Company workshops and events in which I participate and is not limited to the specific event for which I am submitting this form.
